Join Norfolk Community Health & Care NHS Trust on a 12‑month secondment / fixed‑term contract to support the Urgent Community Response (UCR) team. You will provide rapid, high‑quality care for patients in their own homes, working with a strong emphasis on palliative, wound and catheter care, and community nursing skills.
As a registered nurse you will work 7 days a week, including weekends, delivering patient‑centred care during the 2‑hour urgent response window. You will drive quality of life, prevent unnecessary admissions, and support early discharge from hospital. Access to vehicle and a full driving licence are required.
Main Duties and Responsibilities:- Prevent unnecessary hospital admission by assessing and managing patients in crisis situations.
- Assess, plan, implement and evaluate holistic patient care, including palliative, end‑of‑life, wound and catheter care.
- Prescribe suitable care pathways and coordinate with senior nursing and clinical teams.
- Advocate for patients, facilitating choice, empowerment and shared decision‑making.
- Provide evidence‑based therapeutic interventions and maintain accurate clinical records in SystmOne.
- Triage referrals on a rotational basis and collaborate with NHS and community agencies to deliver seamless, integrated care.
- Carry out risk assessments, participate in multidisciplinary meetings and maintain therapeutic relationships with patients and carers.
- Registered Nurse (RN) qualification with current NMC registration.
- Diploma or 1st level degree in nursing or allied health.
- Evidence of continuing professional development.
- Experience in community or acute healthcare settings; training in wound, catheter and palliative care preferred.
- Assessment and mentorship preparation (ENB 998 or equivalent) is an advantage.
- Proven experience in community or acute settings.
- Strong communication, interpersonal and reporting skills.
- Highly motivated, team player with ability to work flexibly and manage stressful situations.
- Capacity to use own initiative, handle confidential information and maintain professionalism.
- Professional appearance and reliability.
- Physical ability to kneel, bend, stoop, and manoeuvre around 5–6 kg.
- Commitment to safeguarding and supporting vulnerable people.
