Job overview

Working as part of a dedicated team of general nurses to offer advice, support and training to Sussex Partnership NHS Foundation Trust services in the delivery of physical healthcare interventions which support the needs of patients living with complex psychological and physical health needs.

The role includes provision of Physical Health and Infection Prevention and Control service for the Trust, offering advice, support and training with elements of audit, improvement plans and associated support and the identification and management of outbreaks of infection.

This role will champion and lead developments in physical healthcare and support frontline teams in the patient environment.

This post offers the Trust's £2,000 Golden Helloor Relocation Incentive for Band 6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).

Main duties of the job

The post holder is an RGN working within their professional boundaries to work collaboratively with SPFT frontline teams to advise and support the delivery of excellent physical healthcare and IPC standards to support excellent quality of care and a safe care environment for staff and patients.

Detailed job description and main responsibilities

  • The post-holder will demonstrate an ability to undertake a physical assessment of patients with complex physical health needs, and will plan and support implementation of care and evaluate effectiveness.
  • The post-holder may be required to develop specialist programmes for patients with complexneeds/conditions in collaboration with multi-disciplinary teams both within and external to SPFT to ensure that the needs of the patient are met.
  • The post- holder will also demonstrate safe clinical decision-making and will provide expert advice, support and guidance to inform the safe and effective care of patients within a mental health/ learning disability setting.
  • The post holder is expected to work collaboratively with PHIPC team members and SPFT clinical teams to meet the needs of patients and support the delivery of care objectives, quality improvement and the implementation of Trust policies and procedures as well as national best practice standards of care.
  • The post-holder will participate in the development and delivery of training materials and resources to inform a high standard of physical healthcare/ IPC within SPFT services.
  • The post-holder will have shared responsibility for management of a caseload of patients across SPFT with physical healthcare needs.

Qualifications

  • Registered General Nurse
  • Post registration experience of working as a Band 6 nurse, or an experienced Band 5 nurse seeking professional development.

Knowledge, Experience

  • Professional knowledge acquired through degree level study.
  • A combination of on the job learning in a specialist area and short courses in specific topic or subject areas or breadth of knowledge gained through a combination of on the job learning in a specific area and short courses in specific topic or breadth of knowledge gained through experience to be able to work autonomously.
  • Experience within acute and/or community care settings
  • Evidence of interest in infection prevention and control
  • Recognises the interdependencies between physical and psychological health cam demonstrate effective ways of working to meet the needs of individual patients
  • Experience of taking responsibility of a team or designated area of work
