Are you an innovative Dietitian seeking a specialist community role with system impact?Join the Provider Improvement and Response Team (PIRT), improving outcomes for residents in care and nursing homes across Staffordshire and Stoke-on-Trent.

This permanent post is initially aligned to a 12-month project supporting care home quality improvement and intensive support. The role may evolve in line with service needs.

Secondment applications are welcome, subject to manager agreement.

Main duties of the job

About the role
You will work autonomously within a multi-disciplinary team, supporting care home providers to improve safety, wellbeing and outcomes. You will undertake holistic assessments, deliver targeted interventions, build staff capability and contribute to admission avoidance.

You will also support system learning, quality improvement, and work with partners across Local Authority, primary care, acute and voluntary services.

• Clinically confident with experience supporting older adults/complex needs
• Community experience, ideally with care homes
• Strong communication and problem-solving skills
• Committed to quality improvement and collaborative working
• Flexible to travel

You will receive clinical supervision via MPFT CHIST and AHP services.

• Degree or equivalent post grad qualification in nutrition and dietetics
• experience of wide range of complex clinical conditions
