We have an exciting opportunity to strengthen our team to work in our unique general practice which cares and treats the population of older people living in care homes in Salford.

By joining our high performing organisation and a multi-disciplinary team including GP’s, Advanced Nurse Practitioners and Specialist Nurses skilled in the delivery of high-quality care, you will be able to enhance your skills further and participate in the education of care home health and social care professionals.

Practice Operating Hours

08.00am – 19.00pm Monday – Friday

08.00am – 17.00pm Saturday – Sunday

This post will provide an opportunity to enhance your skills further and participate in the education of care home health and social care professionals. There are close working links with the hospital and daily input from community geriatricians including multidisciplinary meetings.

Main duties of the job

The post holder will require a variety of duties including Home Rounds and Acute Services.

As the GP you will be responsible for prescribing repeat medications, processing and dealing with patient test results, patient medical reports and examinations, providing end of life care and working with families on advanced care planning.

The post holder will make autonomous decisions in relation to presenting problems, whether on weekly home round review or acute on the day issue.

The post holder will record clear and contemporaneous I.T. based consultation notes to agreed standards.

The post holder support admission avoidance with advanced care planning, helping individuals live longer, healthier and more fulfilling lives.

The post holder will collect data for audit purposes and participate in the delivery of the Quality and Outcomes Framework, Local Enhanced Services, Clinical Commissioning Incentive schemes and ensure compliance with the practices Key Performance Indicators.

The post holder will prescribe in accordance with locally agreed or national guidelines and undertake all the regular duties and responsibilities associated with a GP working within primary care.

Qualifications

  • Full registration with the General Medical Council (GMC) with a licence to practise.
  • Certificate of Completion of Training (CCT) in General Practice
  • Inclusion on the NHS England GP Performers List (or eligibility for inclusion before commencing employment)..

Professional Registration

  • Vocationally trained and accredited GP with MRCGP
  • Registered on a performers list

Knowledge, Training & Experience

  • Experience of caring for older people out of hospital
  • Excellent communication and interpersonal skills
  • Strong levels of IT proficiency and familiarity with primary care clinical systems (i.e.EMIS). Experience of using the Microsoft suite of programmes.
  • Demonstrate a willingness to work in a multi-disciplinary team environment
  • Acting as deputy to the GP Clinical Lead, supporting clinical leadership, governance, service development, and multidisciplinary team working.
  • Up to date knowledge and interest in national primary care strategies and ideas on how to implement these for the development of the Practice.
  • Ability to focus on long-term strategic goals as well as short-term issues/details.

Skills & Abilities

  • 6 months experience in Care of the Elderly, and 6 months experience in specialist older people’s mental health or equivalent.
  • A good understanding of the operation of the Quality Outcomes Framework process (QOF) and how this relates to the elderly.
