Job overview
To assist the senior team in providing care and physical rehabilitation to people who require short team period of recovery in their own homes following an acute event or deterioration that may or may not have resulted in an acute hospital stay. The aim is to provide person centred rehabilitation to enable patients to reach their maximum potential and live as independently as possible.
Main duties of the job
- To work as part of the multidisciplinary team and when necessary on own initiative.
- To provide direct care and rehabilitation to the patients served by the team.
- To act in a manner that protects and maintains the patient’s sense of dignity and respect, while observing their individual religious and cultural practices.
- Encourage patients to participate in activities of daily living and facilitate their active involvement in therapeutic activities.
- Assist the patient work towards to rehabilitation goals to enhance independence and wellbeing.
- Ensure active involvement in manual handling of patients as required – in line with the Trust’s Manual Handling Policy.
- To work within the guidance of the Infection Control Policy and wearing appropriate protective clothing at all times.
- Observe record and report changes in the patients’ health and environment to senior staff.
- To be familiar with the steps to be taken in medical emergencies and act accordingly.
- Complete observations including urinalysis and blood sugar monitoring as required.
- Assist the senior team with prelim visits, first visits and review visits to patients.
- Work directly with the therapists within the team to support them providing effective assessment and treatment including in the in‑patient beds if necessary.
- Assist in planning work together with the senior team.
- Assist in contributing to care planning and risk management plans.

Detailed job description and main responsibilities

- Act as direct support to senior members of the team including the Team Leader, associate practitioners, nurses and therapists to ensure the effective service provision.
- RESPONSIBILITY FOR PATIENT/CLIENT CARE
- Encourage patients to participate in activities of daily living and facilitate their active involvement in therapeutic activities.
- Assist the patient work towards to rehabilitation goals to enhance independence and wellbeing.
- Ensure active involvement in manual handling of patients as required – in line with the Trust’s Manual Handling Policy.

Person specification
Knowledge/education/qualifications
- Good standard of education to GCSE or equivalent
- Completed Care Certificate
- NVQ3 in Health and Social Care
Skills/experience
- Experience providing care in a Community Setting
- Experience in rehabilitation with a positive approach to rehabilitative care
- Experience with working with Stroke patients
- Previous experience of System one
Personal Qualities
- Holds current UK driving licence
- Access to vehicle to use for work purposes
