An exciting opportunity has come up for an enthusiastic, competent and ambitious person to join the Thoracic Surgery Team as our Thoracic Cancer Pathway Co-ordinator and Schedular.
We are recruiting for a Cancer Patient Navigator who shares our ambition and vision to deliver the best patient focused care and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.
To support the implementation of the NHS Cancer plan by providing dedicated and effective support to multi-disciplinary teams, working with team members to facilitate the delivery of high quality care for cancer patients.

Main duties of the job
- To navigate and support patients through the cancer pathway using own judgement and initiative, utilising specific departmental and patient administration systems, ensuring strict adherence and compliance to all Trust Policies and procedures.
- To co-ordinate appropriate investigations required, signpost patients to appropriate support and assist with ‘Making every Contact Count’ to ensure the delivery of patient care is of the highest standard and those patients and their families are treated with dignity and respect.
- To utilise iPM/iSOFT/CRIS and other systems/databases to track patient and upload relevant cancer data.
- To be able to utilise somerset cancer register, blue spier to tract patient could be added.
- To help maintain existing databases of patients coming through the pathway.
- To provide accurate and timely updates for weekly Cancer Services Meetings
- To schedule and communicate to patients regarding the management of their operation dates.

Detailed job description and main responsibilities

- Coordinate care for patients coming through the 2WW Pathway.
- Work with a range of clinical and non-clinical staff in the cancer pathway.
- Provide and receive complex information about patient care and interpret the patient plan for other clinical and non-clinical staff.
- A key aspect of this role involves daily communication with patients, relatives and carers and other health professionals. The information and nature of communication required is sensitive due to the nature of cancer and therefore communication in this context requires a high degree of empathy, understanding, diplomacy, honesty, and integrity.
- To have a good knowledge and understanding of the Thoracic Cancer Pathway and an understanding of the radiology tests arranged, and further tests that may subsequently be requested.
- To provide non-clinical advice and information using good communication skills to navigate patients through what will be a time of great anxiety and where patients may be highly distressed.
- Assist with requesting extra investigations as required e.g., blood tests.
- Following up and tracking results and acting on results in a timely, appropriate manner.
- Support the delivery of patient education using ‘Making every Contact Count.
- Provide basic advice and refer on or signpost to other sources of support.
- Co-ordinate and organise appointments and assessments as required and to exchange that information clearly and concisely dealing with anxiety, cultural or language difficulties.
- Plan and organise complex programme for co-ordination of patient pathway.
- Co-ordinate handover with other Teams to facilitate safe and effective transition of care between services to provide seamless support for patients.
- Facilitate communication between different clinical services to provide speedy service for patients with suspected cancer.
- Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.
- To ensure that high standards of documentation and care planning for patients are maintained.
- Coordinate care for patients coming through 2ww pathway and national optimum lung cancer pathway, and targeted lung health check.

Person specification
Qualifications
- NVQ Level 3 in Business and Administration or equivalent level of qualification/ equivalent demonstrable experience
- Recognised work processing / typing qualification.
- Willing to undertake further learning and development courses
- Previous demonstrable experience of staff development and performance management.
Attributes and Abilities
- Able to use own initiative and work without supervision
- Ability to communicate both verbally and non- verbally on a daily basis with people at all levels
- Ability to facilitate change within the work area
Knowledge & Experience
- Relevant health or administration demonstrable experience
- Demonstrable experience within a similar role or equivalent
- Proven knowledge of National Cancer Waiting Times targets Awareness of Improving Outcomes: A Strategy for Cancer and other national cancer policies Proven knowledge of Thoracic Cancer Pathway and potential interventions and terminology
