Job overview
This role is patient facing, supporting the staff, patients and multi-disciplinary team who are within the hospital. The Practitioner role is very clinically and quality focussed and will be responsible and accountable for ensuring standards of care are maintained and improved overnight, at weekend and bank holidays, in conjunction with the CSM who will support team allocation. Experience of managing complex patient groups such as end-of-life patients, dementia, delirium, infection prevention and those at risk of harm from pressure tissue damage. The CSP will possess a high level of clinical skills and have the ability to develop staff working during the out-of-hours periods to develop their own skill sets.
The Clinical Site Practitioner will support improvements in patient safety and patient experience as well as evidenced based nursing practices, leading by example, motivating and empowering other staff to use strategies to provide the best care. Clinical Site Practitioners are highly visible and accessible for patients, visitors, and staff and communicate with the operational teams to promote safe patient flow, where possible, within the hospital overnight, weekends and bank holidays ensuring patients are cared for in the right environments. Close working relationships with the Site Operations Team, Critical Care
Outreach Team and specialty medical teams will ensure a seamless and resilient service is provided to patient, staff and visitors at all times.

- High standards of care are set and delivered
- Patients are treated with compassion, dignity and respect, placing value on diversity
- Patients are cared for in a clean environment
- Patients are managed in line with practices that support the deteriorating patient
- Best practice in infection prevention control is practised
- Monitor and drive CQC fundamental standards and compliance

Education and Qualifications
- Registered Practitioner (with Experience in Acute Care)
- Holds the Adult Acute Care Module (or equivalent)
- Holds Mentorship qualification or acts as Practice Supervisor/Assessor
- Evidence of post-registration study at Level 6 or 7 or equivalent experience
- Holds Clinical Assessment Module (or equivalent)
- Holds Non-Medical Prescribing qualification Module (or willing to undertake)
- Holds IR(ME)R qualification and experience of requesting X-rays
Experience
- Minimum 2 years’ experience at Band 6 level in an acute care setting
- Experience of managing and/or supervising clinical teams
- Experience of managing rosters and staff allocations
- Extensive experience of core clinical competencies such as venepuncture, IV cannulation, 12-Lead ECG’s, NGT insertion, catheterisation
- Demonstrable experience of managing holistic, complex end-of-life care and decisions involving patients and relatives
- Confidence in setting up and monitoring medical devices such as infusion pumps, syringe drivers and other core devices
- Demonstrable experience in nursing the acutely unwell patient
- Experience of managing acute respiratory patients including the use of NIV, CPAP, tracheostomy, or willingness to undertake additional training and competencies including Critical Care upskilling sessions
- Awareness of current developments in healthcare and professional nursing and guidelines development.
Knowledge
- Demonstrates effective leadership skills
- Demonstrates the multi-disciplinary/multi agency approach to patient care demonstrating the ability to advocate for patientsEvidence of working within the clinical governance framework
- Ability to demonstrate comprehensive knowledge of issues including pressure tissue damage, sepsis, dementia, delirium and challenging behaviours
- Extensive knowledge of medicines optimisation and medicines management including parental and intravenous management
- Professional and confident communication skills
- Be up to date with Immediate Life Support (ILS)
- Be up to date or willing to undertake training on Paediatric Intermediate Life support (PILS)
- Ability to liaise with a wide range of professionals
- Ability to analyse complex problems and successfully implement workable solutions
- Have attended Critical Care upskilling sessions/course
- Hold a valid Advanced Life Support (ALS) qualification
