We are seeking to appoint a dynamic and enthusiastic candidate to the role of Patient Pathway Coordinator, located at Cedar House, within our St. Michael's Community Hospital site within the Specialist School Nursing team.
The appointed candidate will provide a high-level administrative service to the team of nursing staff to ensure the best level of care can be provided to all patients and service users who come into contact with the team. Training will be provided as required, and this role is a perfect opportunity for candidates wishing to pursue a career in the NHS within Operations.
The post is based in the Special school’s Nursing team (SSNT) which covers several special schools in the borough of Enfield, currently the special schools have a total of 17 sites and special schools support CYP with a range of needs from several physical disabilities to mental health and communication concerns.
The function of the team will be to provide care to children with nursing needs in the schools, but also to coordinate care plans and teach education staff/parents or carers in manging the health care needs of their child or young person.

Main duties of the job
The postholder will join a high performing administration team maintaining all patient pathways within the Enfield Community Services – Specialist School Nursing team, working alongside our Consultant Community Paediatricians, Community Nursing team, Speech & Language Therapists, and support staff from across the Health Unit.
The postholder is responsible for ensuring the safe and efficient management of the patient throughout their pathway; giving the patient a single point of contact for their appointments, ensuring the patients and carer experience is central to every stage of the patient journey, and ensuring potential breaches of RTT (referral to treatment) waiting times targets are avoided or escalated to the relevant manager.
The postholder will proactively manage and track the progress of all referrals to ensure each stage of the pathway is fully planned and booked, whilst also working with colleagues throughout the Trust to ensure waiting time targets are achieved. They will be required to assist in the data quality and validation, ensuring all patients have recorded pathway start dates, and support the management team to identify areas that require pathway improvements.

Education & Professional Qualifications
- Educated to GCSE Level 4/C or functional skills level 2 or equivalent in English and Maths.
- Qualification in business administration at minimum of Level 2 or equivalent experience.
- Qualifications in IT services.
Experience
- Previous experience working within an administrative role.
- Experience dealing with the public in a workplace setting.
- Knowledge of NHS service provision including the pathway of care
- Knowledge of spreadsheet and data set maintenance and audit.
- Knowledge of key NHS systems including Careflow, DictateIT, CIP, DA Languages, and Envoy Messaging.
- Knowledge of confidentiality and data protection principles.

- Experience of using a full range of IT/Patient administration systems
- Prior experience of patient administration work in a healthcare setting.
