Specialist Pharmacist – Hepatology in London

We are looking for an enthusiastic and motivated pharmacist to join our Hepatology Pharmacy team to help deliver a progressive and forward-thinking pharmacy service to our liver patients. The postholder does not require specific experience in hepatology but they need a willingness to learn; training will be provided. They will work with the Principal Hepatology Pharmacist to lead, manage and develop a high-quality, patient-focused clinical pharmacy service for hepatology patients. This varied role provides an opportunity to play a pivotal role in the management of both inpatients and outpatients with a range of hepatological conditions. They will ensure high levels of patient safety and experience and ensure clinical and financial metrics are met.

The postholder will contribute to the World Health Organisation's Hepatitis C elimination target of 2030 by working in outreach hepatitis C clinics in local Drug and Alcohol centres and community locations. They will take part in testing events with the Viral Hepatitis Team to identify patients with hepatitis C for treatment. The postholder will promote the field of hepatology to colleagues and junior staff through education and training and have the opportunity to participate nationally in the British Hepatology Pharmacy Group. The St George’s Hepatology Team have achieved Improving Quality in Liver Services (IQILS) level 2 with the hepatology pharmacy team getting a special mention for the exemplar service provided to our patients.

Main duties of the job

• Working as an integral part of the STHepNet Viral Hepatitis Team to provide hepatitis C treatment to patients in secondary care and the wider community.
• Providing a clinical pharmacy service to hepatology inpatients, working as part of the MDT in consultant-led ward rounds.
• Providing and developing pharmacist-led hepatology outpatient clinics using independent prescribing.
• Ensuring safety, patient experience, clinical and financial metrics are met.

Qualifications

• B. Pharm / M. Pharm / BSc (Pharmacy) or equivalent.
• Professional registration with the General Pharmaceutical Council (GPhC).
• Postgraduate Diploma in Clinical Pharmacy or General Pharmacy Practice (or equivalent).
• Mandatory CPD to maintain fitness to practice.
• Registered non-medical prescriber (or willingness to work towards this).
• Qualification in advanced clinical diagnostics and assessment.
• Membership of the Royal Pharmaceutical Society (RPS).
• Core Advanced Pharmacist Credentialing.
• Formal leadership and management training.
• Membership of the British Hepatology Pharmacy Group (BHPG).

Experience

• Substantial post-registration experience as a clinical pharmacist practitioner.
• Experience of directorate work including drug expenditure reporting, formulary management, DTC applications etc.
• Experience in delivering training and education at undergraduate and postgraduate level.
• Experience of working within a role involving multidisciplinary communication.
• Experience of pharmacist-coordinated discharge medication, patient medication history taking.
• Experience of identifying and managing risk in a clinical specialty.
• Experience of consultant-led ward rounds.
• Experience of supervising and managing individual staff and a team.
• Experience of co-ordinating and leading on clinical audit.
• Experience of being an educational and practice supervisor for students and trainee and junior pharmacists.
• Specialist clinical pharmacy experience in a relevant area.
• Experience of acting as a designated prescribing practitioner and/or clinical supervisor.
• Experience of using clinical audit and research to improve practice.
• Experience of presenting at directorate or equivalent level.
• Ability to critically analyse and apply research to the clinical setting.
• Experience in pharmacist-led clinics.

Skills

• Clinical pharmacy skills at Diploma level.
• Demonstrates a level of clinical reasoning and judgement.
• Experience working with High Cost medications and utilising electronic funding systems, e.g. Blueteq.
• Demonstrates excellent written and verbal communication skills.
• Ability to influence senior pharmacy and medical staff, and the multidisciplinary team.
