Job overview
The post holder will have a specialist clinical role with responsibility for planning, managing and coordinating the care of people with long term conditions within a defined caseload. The postholder will use case management techniques to reduce unplanned hospital admissions. Inherent in the role is the requirement for the post holder to undertake training to develop skills in medicines management, care co-ordination, managing cognitive impairment, supporting self-care, management and leadership skills in line with the Department of health competency framework for Case Managers.
Main duties of the job
We currently have vacancy for a band 7 position heart failure nurse specialist with full support and training available for this rotational post between community and acute heart failure teams
We are seeking an innovative, highly motivated forward thinking nurse who has the ability to manage a busy caseload on a daily basis using expert clinical knowledge and judgement whilst providing support to staff. You will be expected to have both a ‘hands on' approach in providing high quality care and have a visible and approachable presence on the wards for inpatient reviews also managing a clinic caseload
The successful candidate will be supported by the ward manager and matron.

Detailed job description and main responsibilities
This is a rotational band 7 post rotating between community and acute heart failure teams, experience in cardiology is required and full support and training will be provided by our experienced and passionate heart failure teams.
The role consists of inpatient reviews, titration of medications, clinic management and home visits. You work collaboratively with the existing Heart Failure team, and our wider cardiology, acute and primary care colleagues. Evidence-based care will be inherent in your practice, ensuring that best practice is disseminated throughout the clinical areas.
Person specification
Qualification & Training
- Registered Nurse 1st level registration with NMC
- First level degree, working towards/ equivalent experience
- Extensive experience in cardiology
- Evidence of continuing professional development.
- Ability to work autonomously and within multidisciplinary teams.
- Non-Medical Prescriber or working towards
- Leadership training.
- Teaching qualification.
- Additional long-term condition management training.
Experience & Knowledge
- Minimum 5 years post-registration experience within cardiology
- Experience in acute or community settings
- Experience working autonomously and collaboratively.
- Evidence of innovation in clinical practice
Skills & Ability
- Ability to manage distressing situations with empathy.
- Clinical governance and risk management knowledge.
- Ability to identify high-risk patients.
