Job overview

Mole Valley Neighbourhood mental health team is seeking an enthusiastic and experienced Senior Mental Health Practitioner to join our in the Brief Interventions pathway will work closely with GP Practices. This role is 33.75hrs can be spread in 4 or 5 days a week.

This rewarding role involves providing high-quality, person-centred care and support to adults with established mental health needs, helping them achieve recovery goals and maintain independence within their local community. The post holder will manage a complex caseload, undertake comprehensive assessments, deliver psychosocial interventions, and work collaboratively with service users, carers, and partner agencies to ensure effective and coordinated care.

Subject to reasonable adjustments in respect of any disability, candidates must possess a valid driving licence and be able to drive and have the unrestricted use of a car or motorcycle for work at all times including community based work.

Main duties of the job

You will take a key role in developing and shaping a major new and transformative approach to providing mental health care within the community.

Will be responsible for working closely with General Practitioners and the primary care team to provide assessments, brief interventions, and care navigation for people with significant mental health issues living in the locality.

You will need to work closely with local mental health services to support an easy in and out approach between secondary and primary care.

Qualifications

  • Professional Qualification in Mental Health Nursing or Occupational Therapy or Social Work
  • Professional registration such as NMC/HCPC/SWE

Experience

  • Further professional training or experience to post graduate diploma level or equivalent experience of looking after people in the Community
  • Clinical supervision training
