Job overview

The post-holder will work as part of a dedicated highly-specialist complex colorectal cancer multidisciplinary team which offers a tertiary service to individuals diagnosed with locally-advanced and recurrent rectal cancers and colorectal peritoneal metastases.
They will be expected to demonstrate a high level of practice in both her/his direct care of patients and in her/his ability to plan and develop services to support complex colorectal cancer care. Through the key skills of leadership, innovation and clinical expertise s/he will facilitate and deliver care which is patient-centred, safe and effective.
The post holder will play a key role in facilitating and co-ordinating the patient pathway. By working collaboratively with clinicians, administrators and service managers they will promote, develop, review and redesign care pathways which are responsive and relevant to complex colorectal cancer patients’ needs.

Main duties of the job

Implement processes along the patient pathway to ensure it is managed
efficiently, smoothly and in accordance with targets and agreed timescales.

Working with clinical areas to facilitate a co-ordinated and streamlined pathway of care which revolves around the patient and their family’s individual needs.

Provide robust timely and comprehensive assessment and personalised care planning.

Support the delivery of pre- and post-operative nursing care and personalised discharge planning.

Co-ordinate and deliver comprehensive arrangements for communicating with patients and their local teams (who may be located far away).

Support facilitation of clinics and multidisciplinary team meetings to meet the standards and requirements of specialist commissioning and the national framework.

Detailed job description and main responsibilities

  • Registered Nurse Degree level
  • NMC registration
  • Evidence of continued professional development relevant to clinical area
  • Recognised teaching qualification or working towards
  • Masters degree or working towards
  • Fundamentals in Cancer Care (Level 7 module) or equivalent

Knowledge and experience

  • Relevant post registration experience at band 6 or above in oncology or tumour site related clinical area
  • Previous experience of teaching and developing teaching materials
  • Demonstrate evidence of highly developed clinical practice with experience of dealing with complex clinical scenarios and interpreting complex needs of patients
  • Previous experience working as a Clinical Nurse Specialist
  • Experience in audit and research
  • Ability to lead and influence change
  • Experience and knowledge in colorectal surgery
  • Experience in running nurse-led clinics
