Hazelwood are looking for an Assistant Practitioner, who has a passion for providing high quality patient focused care who have a flexible attitude towards work and a desire for new challenges.
On Hazelwood you will care for patients with respiratory illnesses and general medical conditions, you will learn to care for patients with long term respiratory conditions such as COPD, Interstitial Lung Disease, type 1 respiratory failure and type 2 respiratory failure.

Main duties of the job
General duties will include , physiological observations and investigations. You will develop an in depth knowledge of , oxygen therapy, blood gas interpretation, wound management, chest drain and fluid balance management and provide personal care to patients.
As a Band 4 you will receive direct support from senior nursing colleagues to develop and enhance your skills. Enabling you to nurse a cohort of patients and be responsible for their care.

Detailed job description and main responsibilities
Contribute to the assessment, implementation and evaluation of care to meet patients’ needs in accordance with agreed local and national standards and evidence based guidelines under supervision.
Support and provide patient education and health promotion.
Support the delivery and co-ordination of care, through appropriate professional practice.
Contribute to the management of risk and maintain patient safety, conforming to health, safety and security legislation, policies, procedures and guidelines.
Communicate all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamless care.
Maintain accurate and complete records of all care activities, in a timely manner, adhering to local and professional standards for documentation.
Person specification
Knowledge & Experience
- Significant experience working in a healthcare environment as part of a team.
- Proven competency of assisting with a core range of physical measurements.
- Experience of using clinical systems –e.g. Quadramed
Qualifications & Education
- Registered Nursing Associate on the NMC register Nursing Associate foundation degree qualification
- GCSE Grade A-C or Grade 4 or above in Maths & English or skills level 2 Maths & English or equivalent
- Good Information Technology skills
