The post-holder will be managed by the ward manager F3, and to ensure the patient’s journey through the hospital is as smooth as possible without any unnecessary delays. Most importantly, administrating and managing patients’ discharge. This role is front of house which requires the post holder to maintain a professional and courteous manner when dealing with colleagues and members of the public. The role of Care Co-Ordinator fulfils a link between Consultants, Medical Teams, Ward Nursing Staff, Social Care Services, external agencies, patients and their relatives.

Main duties of the job include:

  • Essential support to the relationship between Wards, Clinical Site and patient flow team.
  • Communicating the immediate position on bed occupancy and discharge information on the ward.
  • Supporting the operational management team by providing information and expertise on patients’ discharge and delivering on the operational priorities.

The West Suffolk Hospital in Bury St Edmunds provides acute and secondary care services including emergency department, maternity and neonatal services, day surgery unit, eye treatment centre, Macmillan unit and children's ward. It has 500+ beds and is a partner teaching hospital of the University of Cambridge.

Adult and paediatric community services, provided in collaboration with West Suffolk Alliance partners, include a range of nursing, therapy, specialist, and ongoing temporary care and rehabilitation.

Person specification includes:

  • Achievement through study to level 3 or above (NVQ, GCSE A-C or equivalent, 1st Diploma, Apprenticeship).
  • Level 2 Math’s & English qualification or equivalent.
  • Ability to work on own initiative and within a team.
  • Understanding the importance of patient confidentiality.
  • Able to demonstrate skills that ensure tasks are completed accurately and within a given timeframe.
  • Clear and precise communication skills.
  • Understanding Trust Policies with regards to requests for leave.
  • Able to work under pressure and meet deadlines.
  • Good interpersonal skills and attention to detail.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
