About Transform Housing & Support

What You'll Be Doing
- You'll work as part of a dedicated team, supporting clients daily to resolve problems and achieve positive outcomes.
- Your role combines meaningful support work with practical housing management.
- Act as keyworker to named clients, helping them live as independently as possible.
- Assist clients to meet personalised support plans and access community services.
- Carry out housing management activities including rent collection and tenancy management.
- Conduct risk assessments and health and safety checks.
- Report maintenance issues and ensure client safety.
About You
- You’ll need passion and resilience to support and empower Transform clients to develop their independence and daily living skills.
- Ability to develop non-judgmental professional relationships with clients.
- Strong time management skills, delivering outcomes and meeting deadlines.
- Computer literacy (Excel, Word, Outlook and Internet).
