We are inviting expressions of interest for Social Care Assessors to join the new Adult Support and Prevention Hub. This is an exciting opportunity to contribute to Trafford’s early help and prevention pathways at a time of positive change and development.

As Trafford’s first point of contact for residents, the team delivers a whole-system, preventative model that brings together social care practice, occupational therapy and strong partnership working. Our flexible access routes including online, telephone, email and in-person support are designed to make it easier for people to get the right help at the right time. The team has already achieved important milestones, including improved access to aids and adaptations, Telecare, admission avoidance support, community activities and short-term interventions that help people stay independent for longer.

You are committed to person-centred, strength-based practice and motivated to make a positive difference to adults, carers and families. You will have experience working in a social care, health or community setting, with the ability to carry out proportionate assessments, develop clear support plans and work confidently with risk.

You communicate well, build positive relationships and work collaboratively with colleagues, partners and voluntary sector organisations. You are organised, able to manage a varied workload and confident using IT systems to maintain accurate, timely records. You bring curiosity, empathy and a proactive approach to helping people stay independent and connected to their communities. You are reflective in your practice, open to learning and committed to delivering safe, high-quality support as part of Trafford’s Adult Support and Prevention Team.

An enhanced DBS Check with adults barred list will be required. You will be required to drive in this role. You will be asked for a copy of your driving license as part of the recruitment process. Willing and able to travel to work at sites within and outside the Trafford Borough. Hybrid work style; working a minimum of 2 days in our offices. Unsocial hours/weekend work may be required.
