The Trafford College Group is seeking two dedicated SEND Transition Coordinators to support learners aged 16-24 as they move into college life.
Based at Altrincham or Cheadle campuses, you will work with students, families and partner agencies to enable successful transitions and positive outcomes.
You will manage SENCO datasets, develop person-centred plans, and collaborate with Careers, Admissions and Learning Support teams to tailor support, enabling learners to reach their potential.
