Location: Cheadle Campus (although requirements across the Group may require cover at other sites). The Trafford & Stockport College Group (TSCG) has an opportunity for an experienced Transition Coordinator to join the Inclusion team to help us achieve our vision of unlocking potential and fostering success. This is a key role in supporting the College’s transition programme, working closely with schools, local authorities and other external agencies to ensure learners with EHCPs experience high quality, personalised and seamless transition into the College.
The Role: They will have the confidence to develop rapport with the young people, their families, carers and professionals, in line with the College’s duties under Children and Families Act and the SEND Code of Practice regarding transition. The postholder will promote the College’s commitment to the Department for Education Preparation for Adulthood agenda, by providing advice on the College’s curriculum and support offer, which allows learners to make informed decisions about next steps. This will be supported by the successful candidate working closely with Careers, Guidance and Admissions teams and curriculum areas to advise on learners’ support requirements and act as an advocate for the student and family if appropriate.
