Traditions Home Health Services is seeking compassionate, skilled, reliable, and experienced Hospice-trained Certified Nursing Assistants to assist clients on Hospice residing in the Plymouth area.
Available Shifts:
- Morning: 8am - 12pm
- Evening: 5pm - 10pm
Job Responsibilities:
- Assist with daily living activities such as toileting, incontinence care, bathing, dressing, grooming, nail care, oral hygiene, and skin integrity maintenance.
- Monitor vital signs, facilitate daily exercises, and perform range of motion exercises as per the care plan.
- Prepare healthy meals and ensure adequate hydration according to the care plan.
- Follow infection control protocols.
- Administer medication reminders and assist with eye drops, ointments, creams, and ear drops.
- Perform housekeeping tasks to maintain a safe and clean environment.
- Engage clients in stimulating activities to promote joy.
- Provide transportation for outings, errands, and appointments.
- Proficient in dementia care.
- Document shift activities digitally using a smartphone.
- Report concerns promptly to the Office Support Team.
