We are seeking a dedicated and compassionate Triage Nurse to join our healthcare team. In this role, you will be the first point of contact for patients, ensuring they receive the appropriate care in a timely manner. Your ability to assess patient needs, prioritize urgency, and communicate effectively with patients and healthcare teams will be key to your success. Responsibilities

  • Assess and evaluate the urgency of patient symptoms via phone, in-person interactions, or virtual consultations.
  • Utilize clinical protocols to prioritize patient needs and determine the appropriate level of care required.
  • Provide clear and compassionate communication with patients regarding their conditions, care instructions, and next steps.
  • Document patient assessments and medical information accurately in the appropriate medical records system.
  • Collaborate with physicians, nurse practitioners, and other members of the healthcare team to coordinate care plans tailored to patient needs.
  • Monitor follow-up protocols to ensure continuity of care and optimal patient outcomes.
  • Stay informed of the latest advancements in healthcare triage practices and maintain compliance with all clinical safety and ethical standards.

Qualifications

  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Bachelor of Science in Nursing (BSN) preferred, but not required.
  • At least 2 years of experience in nursing, with previous experience in triage, emergency, or acute care settings preferred.
  • Strong clinical assessment and critical thinking skills.
  • Exceptional communication and listening abilities, with the capacity to manage sensitive conversations empathetically.
  • Proficiency with electronic health record (EHR) systems and technology.
  • Ability to work collaboratively in a fast-paced and diverse environment.

Day-to-Day

  • Answer and triage calls from patients or caregivers, assessing symptoms and determining the appropriate action.
  • Document patient encounters and decisions made during the triage process.

  • Support healthcare providers by ensuring patients with urgent needs are seen promptly and efficiently.
  • Administer follow-up calls to patients as needed, ensuring clarity on care instructions and addressing any additional questions or concerns.
