Job Responsibilities

  • Medication Review and Optimisation: Respond to reception queries to amend prescriptions, re-authorise repeats, clarify doses, clarify products, and provide appropriate alternatives when availability issues arise. Discuss specific patient and prescribing issues with other clinicians as necessary. Identify patients in need of medication review and develop a system of patient invitation with the admin team, seeing patients face-to-face for medication reviews and discussing specific issues. Address medication issues with patients as needed by telephone.
  • Medicines Reconciliation: Review secondary care requests for new medication and raise any queries with the relevant GP. Review and continue repeat medications for new patients who have just registered with the practice and suggest GP medication review where appropriate.
  • Medicines Information/Education: Monitor and inform colleagues about ongoing prescribing issues, including new guidelines, new products, manufacturing and supply problems, and individual errors made by colleagues. Advise on cost-effective prescribing and prescribing budget issues.
  • Prescribing Systems and Policies: Liaise with reception, admin, and clinical staff to discuss, develop, and implement medicines management systems. Work with GPs and the practice manager to review, develop, and implement prescribing policies and strategies. Suggest and design audits related to prescribing targets and monitor the practice's prescribing performance.
  • Working Relationships and Communication Requirements: Communicate information to patients, carers, colleagues, and other agencies in a manner consistent with their level of understanding. Provide complex information to patients/carers and support those receiving it. Practice within a legal and ethical framework that adheres to the GPC Code and PML policies. Maintain confidentiality of information consistent with legislation and PML policies. Be proficient with IT systems, compile reports, and analyse data.
  • Registered Health Professional: All staff who are members of a professional body must comply with standards of professional practice/conduct.
  • Information Governance: Ensure that records created are of high quality, accurate, and compliant with PML's policies and procedures, maintaining confidentiality and security of all information.
  • Confidentiality: Maintain confidentiality of information in accordance with professional and PML policy, accessing information only on a need-to-know basis.
