Primary Care Onsite Clinical Pharmacist (Independent Prescriber) Oxford PCN
Location: Oxford, Oxfordshire

About the Role
We are looking for a highly motivated and qualified Clinical Pharmacist (Independent Prescriber) to join a friendly and supportive GP practice within Oxford PCN. You will play a key role in improving patient care by providing expert pharmaceutical advice, resolving medication queries, and supporting the smooth day-to-day running of the practice. Working as part of a multidisciplinary team, you will contribute to better patient outcomes and improved access to healthcare.
Key Responsibilities
- Resolve medication-related queries for patients and practice staff
- Conduct polypharmacy and structured medication reviews (SMRs)
- Perform face-to-face medication reviews and post-discharge reconciliations
- Provide clinical leadership on medicines optimisation and quality improvement
- Offer expert pharmaceutical advice to clinicians and patients
- Support audits, service improvements, and QOF/CQC workstreams
- Promote safe, effective, and efficient use of medicines
- Educate patients on medication use and disease prevention

Requirements
Essential:
- GPhC-registered Clinical Pharmacist with Independent Prescribing qualification
- Completion of CPPE Pathway or Exemption
- 12+ months experience in a GP practice or primary care setting
- Experience using EMIS or SystemOne
- Living within a commutable distance to Oxford
Additional:
- Understanding of QOF, CQC, and local incentive schemes
- Enhanced DBS check required (in line with Rehabilitation of Offenders Act 1975)
