About the Role

We’re recruiting two enthusiastic and proactive Primary Care Clinical Pharmacists (Independent Prescribers) to join two friendly GP practices within Barnet Primary Care Network (PCN). This is an excellent opportunity for motivated Clinical Pharmacists who want to make a real difference in patient care delivering high-quality pharmaceutical services, optimising medicines use, and supporting the multidisciplinary team to enhance outcomes and patient safety. You’ll play a key role in the smooth day-to-day running of the practices, providing expert clinical support and medication advice to patients and colleagues alike. Hybrid working is available with the balance of onsite and remote work to be agreed.

Key Responsibilities

  • Act within professional boundaries as an Independent Prescriber, supporting both patients and clinicians.
  • Conduct Structured Medication Reviews (SMRs) and polypharmacy reviews.
  • Provide face-to-face and remote consultations with patients.
  • Offer expert medication advice to GPs, nurses, and other clinicians.
  • Support medicine reconciliation and review post-hospital discharge.
  • Contribute to service improvement, medicines optimisation, and quality initiatives.
  • Resolve prescription and medication queries efficiently.
  • Educate patients on medication use and disease prevention.
  • Participate in QOF, CQC, and local incentive scheme workstreams.
  • Lead and support the implementation of safe and effective prescribing practices across the practice.

What We’re Looking For

Essential Qualifications

  • Independent Prescriber (IP) qualification CPPE Pathway or equivalent exemption

Essential Experience

  • 6-12+ months working within a primary care or GP practice setting
  • Experience using EMIS Web
  • Able to commute to Barnet for onsite working days

Desirable

  • Strong understanding of medicines optimisation and clinical governance
  • Excellent communication, problem-solving, and team collaboration skills

This post requires a Disclosure and Barring Service (DBS) check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
