As a Health and Housing Coordinator, you will work alongside hospital discharge teams, patients, families and partner agencies to help people return home safely and maintain their independence.

This is a varied and rewarding role where no two days are the same, combining problem-solving, partnership working and practical support to improve outcomes for vulnerable individuals across the community.

  • Experience supporting vulnerable people in a professional or voluntary capacity.
  • A compassionate and empathetic approach with the ability to build trust quickly.
  • Strong problem-solving skills and the ability to work effectively in a fast-paced environment.
  • Knowledge of housing-related support, welfare benefits, community services, home adaptations or independent living solutions.
  • Excellent communication and IT skills, including Microsoft Office.
  • A full driving licence, access to a vehicle for work purposes and the ability to travel across the locality.
  • Commitment to safeguarding, confidentiality and delivering excellent customer service.

What You Will Do

  • Support patients, families and carers to achieve safe and timely discharge from hospital.
  • Assess barriers preventing individuals from returning home and coordinate practical solutions.
  • Conduct home visits before and after discharge to assess safety and suitability.
  • Work closely with health professionals, local authorities and community organisations to access the right support.
  • Help prevent hospital admissions by supporting individuals in the community and attending A&E pathways where required.
  • Maintain accurate records, manage cases effectively and ensure safeguarding concerns are acted upon promptly.
  • Promote independence, wellbeing and positive outcomes for customers and patients.

This is a full time 15 month fixed term position working 35 hours a week and is based at Tunbridge Wells Hospital in Pembury.

This is a hybrid role where we expect all staff to come into their base office 2 days a week (everyday during induction which could be up to 3 months).
