They work closely with the NHS, local authorities, and healthcare professionals to ensure continuity of care and excellent outcomes for their service users.
Role Overview
They are seeking a compassionate, organised, and proactive Care Coordinator to join the growing team. The Care Coordinator plays a vital role in ensuring the smooth delivery of care services by coordinating care packages, managing care rotas, and acting as a key point of contact for service users, carers, families, and healthcare professionals. This role is ideal for someone with strong administrative skills, excellent communication abilities, and a genuine passion for delivering outstanding domiciliary care.
Key Responsibilities
- Care Coordination & Scheduling
- Coordinate and manage care rotas to ensure safe, consistent, and effective care delivery.
- Allocate care workers appropriately based on skills, availability, and service user needs.
- Respond to changes, absences, and emergencies, ensuring continuity of care at all times.
- Service User & Stakeholder Liaison
- Act as a first point of contact for service users, families, care workers, and professionals.
- Communicate effectively with NHS teams, local authorities, and other external partners.
- Support onboarding of new service users, including care package setup and reviews.
- Care Quality & Compliance
- Ensure care delivery aligns with care plans, risk assessments, and CQC standards.
- Support audits, spot checks, and quality assurance processes.
- Escalate safeguarding concerns in line with company and NHS policies.
- Administrative & Operational Support
- Maintain accurate care records, rota systems, and documentation.
- Support the Registered Manager with reporting, compliance, and operational tasks.
- Assist with recruitment coordination, induction, and ongoing staff support.
Skills & Experience
- Essential
- Previous experience in domiciliary care or a care coordination role.
- Strong organisational and time-management skills.
- Excellent written and verbal communication skills.
- Good IT skills, including care management systems and Microsoft Office.
- A caring, empathetic approach with the ability to work under pressure.
- Desirable
- NVQ Level 3 in Health & Social Care (or willingness to work towards).
- Experience working with NHS-commissioned care packages.
- Knowledge of CQC regulations and domiciliary care standards.
