We are seeking an enthusiastic GP or ANP for a unique opportunity in reviewing and managing patients within our two local nursing homes. We can offer up to 5 sessions per week with flexible working hours to ensure adequate cover is provided for our patients within our nursing homes. There may be opportunity for further general clinical sessions within this offer based on experience and interview. We are seeking candidates with a background of experience in nursing home care. This will be a full working week role with sessions spread Monday – Friday. Salary is to be negotiated dependent on background experience and performance at interview.

Main duties of the job

Conduct comprehensive clinical assessments and ongoing care for nursing home residents.

Diagnose, treat, and manage acute and chronic illnesses in line with best practice and local guidelines.

Perform regular medication reviews and optimize prescribing for frail and elderly patients.

Provide palliative and end‑of‑life care, including advanced care planning discussions.

Liaise effectively with nursing home staff, families, and external healthcare professionals.

Maintain accurate and timely clinical documentation in accordance with regulatory standards.

Participate in safeguarding processes and ensure compliance with CQC and local governance requirements.

Contribute to quality improvement initiatives and audits related to nursing home care.

Job purpose

Key Responsibilities

Person Specification

Qualifications

  • Fully qualified GP (MRCGP) or ANP with independent prescribing rights.
  • Current registration with GMC for GPs or NMC for ANPs.
  • Additional qualifications in geriatrics, palliative care, or advanced clinical practice.

Experience

  • Proven experience in providing care within nursing home or care home settings.
  • Strong knowledge of geriatric medicine, frailty, and polypharmacy management.
  • Excellent communication and interpersonal skills, with a compassionate approach.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Experience with electronic health record systems and remote consultations.
