Tower Hamlets is an ambitious, diverse and inclusive borough with high aspirations for all children and young people. We work in partnership with families, schools, colleges, health services and wider partners to build a responsive, person-centred SEND system that improves outcomes and helps children and young people thrive.

We are seeking an experienced and highly organised professional to play a key role in strengthening our multi-agency approach to Preparing for Adulthood and improving outcomes for young people with SEND. This is an opportunity to make a tangible difference by supporting joined-up transition planning and helping partners work together effectively to deliver better experiences and outcomes for young people and their families.

You will bring strong partnership working skills, excellent organisational and administrative ability, and the confidence to work across complex systems with a wide range of stakeholders. If you are passionate about improving SEND Preparing for Adulthood services and want to play a central role in ensuring that transition planning is timely and joined up, with a clear focus on what matters most to young people and their families, we would welcome your application.
