Tower Hamlets GP Care Group is recruiting a SEND Assistant Practitioner to join the 0-5 Health Visiting Service. You will support babies and children aged 0-5 with SEND or complex health needs and work with their families in homes, family hubs, nurseries, GP surgeries and clinics.
Under the supervision of the Specialist SEND Health Visitor, you will carry out developmental reviews, provide 1:1 and group support, and help families access the right services early.
