Main duties of the job

You will be an experienced registered Advanced Nurse Practitioner who, acting within their professional boundaries, will provide care for patients registered at the practice from initial history taking, clinical assessment, diagnosis, treatment, evaluation of care, prescribing of medication and development of management plan. The role will involve care planning and working in the community with the practice's patients who may be housebound, vulnerable adults or residents in care/residential homes. You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. You will work collaboratively with the general practice team to meet the needs of patients, support the delivery of policy, procedures, future practice strategy and provide clinical leadership as required.

We provide services to around 48,000 registered patients, that's a quarter of the population of Bury, and our patients can access services across all four of our sites. Our clinical team is diverse including GPs, a Practice Paramedic, Pharmacists, Advanced Clinical Practitioners, Advanced Nurse Practitioners, Practice Nurses, Nurse Associates, a Physician Associate and highly skilled Health Care Support Workers, working closely with a range of additional roles including an Acute Visiting Service, Mental Health Practitioners and First Contact Physiotherapists. To allow more time to care, our clinical team is underpinned by a very experienced team of receptionists and administrators, as well as a range of in-house corporate functions covering themes such as human resources, information technology, finance, administration and management support and communications.

Job responsibilities

  • Assess and treat patients presenting with acute illness or for review of chronic disease conditions.
  • Undertake home visits for acute illness or chronic disease management. Reviewing care plans and liaising with patients, carers, relatives and other healthcare professionals to support a holistic package of care.
  • Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national, local and practice protocols and formularies.
  • Work with patients and their carers in order to support compliance with and adherence to prescribed treatments.
  • Provide a telephone triage service, assessing patients' health needs and management plans. Implement and evaluate clinical interventions to meet those needs. Record summary of discussion, intervention and actions onto the clinical system.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
  • Support and manage health needs of women presenting for family planning or sexual health consultation.
  • Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
  • Implement and participate in vaccination and immunisation programmes for both adults and children.

Person Specification

Experience

  • Minimum 5 years post registration experience.
  • At least 3 years recent primary care/community nursing experience.
  • Nurse-led management of minor illness, minor ailments and injuries.
  • Nurse-led triage including telephone triage.
  • Home visit assessment of patients.
  • Compiling protocols and clinical guidelines.
  • Leadership in quality initiatives such as clinical benchmarking.
  • Audit.
  • Independent Nurse Prescribing or willingness to undertake and complete nurse prescribing training.
  • Project Management.
  • Working with community development initiatives.
  • Health needs assessment.
  • Sexual Health including fitting/removal of IUCD and LAC.
  • Minor Surgery.
  • Extended & Supplementary Prescriber.

  • Advanced Clinical Practice Skills ANP Qualification or equivalent evidence to demonstrate skills.

  • ECDL.
