Role and responsibilities
- Provide high quality, skilled nursing care and assist the Community Nursing Team Manager in the planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.
- Deliver high quality nursing care to housebound patients with complex needs to enable them to remain in their own homes.
- Work closely with specialist nurses, allied health and social care professionals to meet patient needs without unnecessary attendance in A&E or admission to hospital.
- Work independently but as part of the Community Nursing Team covering a particular geographical area. This will include first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care.
- Be flexible to support a corporate approach to patient care as required.
- Collaborate with integrated rapid response teams, community matrons and social service colleagues to ensure seamless care for patients.
Location, hours and environment
Based at the Heart of Hounslow Health Centre, the service operates from 07:00 to 19:00, seven days a week, with alternate weekends. Shifts available as 07:00β15:00 (early), 08:30β16:30 (day), and 11:00β19:00 (late).
Qualifications and requirements
- Registration as a General Nurse (RGN) with the relevant professional body (UK).
- Band 5 Community Nurse role; able to work across the Hounslow area.
- Full UK driving license with access to a car.
- Excellent communication skills and the ability to work effectively within a team.
- Motivation toward personal development.
