Cardiorespiratory & Post-COVID Specialist Physiotherapist/Nurse
West London NHS Trust is seeking a skilled and motivated Nurse or Physiotherapist to join the Hounslow Community Respiratory Service. This Band 6 role offers an exciting opportunity to deliver specialist, patient‑centred rehabilitation for adults with long‑term cardiac and respiratory conditions, including those diagnosed with Long COVID, across home, clinic, community venues, and virtual platforms.
The successful candidate will provide holistic assessment, rehabilitation, and case management to support patients in maximising independence, functional ability, and quality of life. You will work collaboratively within a multidisciplinary team to support Pulmonary and Cardiac Rehabilitation (PR/CR), Early Supported Discharge (ESD), chest physiotherapy, and specialist interventions delivered through HOSAR.
- Deliver Pulmonary and Cardiac Rehabilitation programmes in line with NICE, BTS, and ACPICR guidance.
- Providing specialist assessment and case management for adults with long‑term cardiac or respiratory conditions.
- Supporting patients with breathlessness, fatigue, anxiety, and functional limitations.
- Delivering chest physiotherapy and oxygen therapy interventions within the community.
