An exciting opportunity has arisen within the Ealing Community Team for People with Learning Disabilities within the Nursing team. The CTPLD is looking for a part time band 6 Learning Disabilities Nurse to join the team. The post is a part time post, of 18.5 hours per week / 0.5FTE. Flexibility can be offered as to working days with the option of some remote working.
The nursing team is a strong, passionate and dedicated team of RNLDs who endeavour to improve the health outcomes for people with learning disabilities across Ealing. The successful candidate will be responsible for implementing the nursing model to assess, plan, implement and evaluate care, ensuring people's physical health needs are met; to support mainstream health services to meet the needs of people with learning disabilities and to support hospital admission and discharges. The successful candidate will have the opportunity to lead on service development projects and implement innovative nursing and multi-disciplinary pathways. You will work alongside the multi-disciplinary team including psychiatry, nursing, psychology, PBS, speech and language therapy, occupational therapy, physiotherapy to ensure a holistic approach is taken to manage health needs. Main duties of the job:
- Undertaking initial nursing assessments
- Care planning and implementation
- Supporting mental capacity assessment and best interest meetings
- Supporting family members, carers and providers
- Health promotion
- Health facilitation
- Implementation and development of care pathways
- Liaison with external stakeholders
Support Service Users To Manage Their Physical Health Needs:
- Supporting GPs to increase annual health check uptake and quality
- Facilitating blood tests via desensitisation programmes
- Epilepsy care plans
- Supporting the dementia pathway
- Symptom management and end of life care planning
- Supporting weight management
- Promoting positive sexual health
- Promoting healthy lifestyle choices
Partnership Working With The MDT:
- MDT Initial Assessments
- Attending weekly referral meetings
- Attending weekly case discussion meetings
- Raising safeguarding concerns
- Reporting incidents
Management:
- Supporting student nurses in the role of practice supervisor and assessor - additional training available
- Attending regular supervision
- Adhering to the NMC requirements including revalidation

Person Specification:
Essential:
- Registered General Nurse, Adult Branch and current NMC Registration
- Experience and evidence of teaching and assessing e.g. Preceptorship/Mentor or working towards
- Commitment to complete advanced physical assessment course/clinical decision making/District Nursing Specialist Practitioner Course
- Evidence of recent CPD
- Ability to work in community setting/home visits
Desirable:
- Advanced clinical skills - ability to assess, diagnose and treat as an independent practitioner minimum level 6 study or similar
- Advanced clinical assessment skills, clinical decision making/District Nursing Specialist Practitioner Course at minimum level 6 study or similar course or commitment to complete
Experience:
- To have an interest in audits and clinical governance.
- Experience of supervising junior staff in the team or work towards developing.
- Demonstrable experience and work qualification, preferably within community care setting.
- Experience of and ability to communicate effectively with variety of partners and stakeholders - verbal, written and electronically
- Ability to problem solve and prioritise.
Knowledge:
