Overview
The Ealing Hospital Discharge Hub has an exciting opportunity for an enthusiastic and motivated individual with expert knowledge of D2A/discharge pathways, discharge planning and experience working within a multi-disciplinary team. Your main role will be to have oversight of day to day patient flow through the discharge hub, coordinate and facilitate P1 to P3 discharges from Ealing Hospital into the community in a safe and timely manner. The role will be based at Ealing Hospital.
The successful post holder will work in partnership with colleagues in the acute sector, social services, Ealing Community Partners, ICB and other community health services to ensure that appropriate patients from the acute setting are assessed and transferred safely to appropriate community-based services in a timely and seamless manner.
Responsibilities
- To work as a core member of the acute based Ealing Hospital Discharge Hub team to build and deliver the shared vision for a hospital discharge service.
- Be responsible to optimize hospital bed utilization and reduce the length of stay by ensuring the timely, safe transfer/discharge of patients to the most appropriate and agreed environment. This will include monitoring the transfer of patients to non-acute rehabilitation centres and addressing delayed transfers of care.
- To act as a pro-active community health service advocate, on behalf of West London NHS Trust, through extensive knowledge of relevant community-based pathways within the area of operation and ability to support effective discharge planning.
Work in close partnership with hospital colleagues and external agencies such as Social Services, ICB, Housing Associations, GPs and voluntary organisations. This will involve attending meetings outside the Trust and sharing Protocols and Care Management Plans with the ASC and other community providers.
