Total Assist is currently recruiting for an experienced Qualified Social Worker to join a specialist private mental health service in Berkshire.

This is an excellent opportunity to work within a specialist mental health and rehabilitation setting, supporting adults with complex mental health needs. You will work closely with a multidisciplinary team to promote recovery, safeguard vulnerable individuals and support positive outcomes.

\n
The successful candidate will have strong assessment and care planning skills, with the ability to manage a varied caseload and work collaboratively with service users, families and partner agencies.

Key Responsibilities

- Manage a caseload of service users with complex mental health needs.
- Complete comprehensive assessments and develop person-centred care plans.
- Contribute to multidisciplinary team meetings and care reviews.
- Support safeguarding procedures and ensure risks are appropriately identified and managed.
- Work collaboratively with healthcare professionals, families and external agencies.
- Support service users with their recovery, rehabilitation and transition into the community.
- Maintain accurate and up-to-date case records in line with professional standards.
- Ensure all practice complies with relevant legislation, policies and safeguarding requirements.

Requirements
\n

- Recognised Social Work qualification.
- Current registration with Social Work England.
- Previous experience within adult mental health services.
- Strong assessment, care planning and case management skills.
- Knowledge of safeguarding, mental health legislation and risk management.
- Excellent communication and multidisciplinary working skills.

Experience within secure mental health, forensic services or rehabilitation settings would be advantageous.
