Leadership & Management: Provide effective clinical leadership and support the wider team to deliver safe, high-quality care.
Quality Improvement: Contribute to quality improvement initiatives, clinical governance and the continuous development of services.
Complex Moving & Handling: Assess and manage complex moving and handling needs, ensuring safe and appropriate techniques and equipment are used.
Assessment: Undertake comprehensive assessments, identifying individual needs, risks and appropriate interventions.
Rehabilitation: Plan, deliver and review person-centred rehabilitation programmes to support functional independence and recovery.
Care Planning: Develop, implement and regularly review individualised care and rehabilitation plans.
MDT Engagement: Work collaboratively with the multidisciplinary team, ensuring effective communication, coordinated care and shared decision-making.
Risk Management: Identify, assess and manage clinical and environmental risks, escalating concerns appropriately.
Documentation: Maintain accurate, timely and confidential clinical records in line with professional and organisational standards.
