Total Assist Recruitment are currently seeking an experienced Qualified Social Worker to join the multidisciplinary team at a specialist private healthcare service in Devon. The service provides rehabilitation and ongoing support to adults with complex mental health needs, with a focus on recovery, rehabilitation and developing independence.
The Role
As a Qualified Social Worker, you will work as part of a multidisciplinary team, supporting individuals throughout their rehabilitation journey. Your responsibilities will include:
- Managing a caseload and completing social care assessments
- Supporting care planning, discharge planning and transition into the community
- Completing risk assessments and safeguarding work
- Liaising with local authorities, community teams and other external agencies
- Attending MDT meetings and contributing to multidisciplinary care planning
- Supporting individuals with housing, benefits, advocacy and access to community services
- Maintaining accurate and timely clinical and social care records
- Working collaboratively with patients, families and the wider professional team
Requirements
- Qualified Social Worker with a recognised Social Work qualification
- Current registration with Social Work England
- Previous experience working with adults
- Experience within mental health, rehabilitation or inpatient settings is desirable
- Strong understanding of safeguarding, risk management and social care legislation
- Excellent communication and report-writing skills
- Ability to work confidently as part of a multidisciplinary team
- Enhanced DBS or willingness to obtain one
