Role summary
The Integrated Reablement (IR) Service is seeking to recruit a full time Assistant Team Lead who will provide support and management oversight for staff within Torfaen's domiciliary care and reablement teams.
About the IR Service and Ty Glas y Dorlan
The IR Service is multifunctional & provides short-term support and guidance for people living in Torfaen to maintain and improve independence in their own homes or within Ty Glas y Dorlan. The service is flexible and responsive and provides practical help, personal care and support in a way that is focused on the individual and is based on 'what matters' to them. Ty Glas y Dorlan is our wellbeing hub at the centre of Torfaen's respite and Reablement/Enablement offer. It provides a mix of short stay accommodation with support, enabling a person centred recovery and reablement plan of intervention which can flex up and down. It can provide step up from the community to avoid hospital admission and step down from hospital to provide a safe space to recover.
Responsibilities
We are looking for an enthusiastic person who will make a significant contribution to supporting the Team Lead in the development and flow of individuals through the IR Service. A key responsibility will be coordinating and managing admissions into Ty Glas y Dorlan, working closely with individuals, carers/families and other colleagues within Adult Services and health care professionals. The successful candidate must possess good communication and strong leadership skills, be enthusiastic, proactive, creative and forward-thinking with a genuine commitment to working towards achieving the best outcomes for individuals which are person centred, outcome focussed and encourages positive risk taking. The successful candidate will be flexible, motivated and committed to put the individual's strengths, voice, choice and control at the centre of any interventions and must be able to work effectively as part of a team or on their own initiative. Qualifications and requirements
The successful candidates will hold a management/social work/OT or other equivalent professional qualification. They must also hold the Level 5 Diploma in Leadership for Health and Social Care Services (Adults Management) or have a willingness to obtain this qualification.
