The role supports 0–25-year-olds with SEND, coordinating Education, Health and Care Plans with families, schools, and health/social care partners.
Responsibilities include interpreting assessments, co-producing plans, meeting statutory timescales, and maintaining case data using management systems.
Two positions available: 22.2 hours fixed-term and a full-time permanent role.
