POSITION SUMMARY
As the Case Manager of the Winchester Rescue Mission, you will hold a pivotal role responsible to provide individualized support to clients, helping them navigate and access services that lead to stability and self-sufficiency.
Reporting directly to the Programs Director, you assess client needs, develop and implement case plans, and coordinate with community resources to ensure clients receive holistic support.
This position will ensure alignment with the organization’s mission statement and core values of hospitality, ownership, passion, excellence, faith, unity, and learning.
KEY RESPONSIBILITIES
- Client Assessment and
Support: Conduct comprehensive assessments to identify client needs, strengths, and goals.
Develop and implement personalized case plans tailored to each client’s needs.
Provide ongoing support and guidance to residents, including crisis intervention and emotional support.
Monitor client progress and outcomes, adjusting plans as necessary to promote long-term self-sufficiency.
- Service
Coordination: Connect clients with appropriate community resources, such as housing, employment, Social Services, mental health, and healthcare services.
Monitor and evaluate the effectiveness of services provided to residents.
Maintain accurate and up to date case records, documentation, and progress notes.
- Advocacy and
Referral: Advocate on behalf of clients to access necessary services and resources.
Facilitate referrals to external agencies and support services.
Assist clients in understanding and navigating complex systems, including public assistance programs.
- Program
Development: Contribute to the development and improvement of case management programs and services.
Collaborate with other team members and community partners to enhance service delivery.
Participate in staff meetings, training sessions, and professional development opportunities.
- Compliance and
Reporting: Ensure compliance with organizational policies, procedures, and regulatory requirements.
Prepare and submit reports on case management activities, client progress, and program outcomes.
Maintain confidentiality and adhere to ethical standards in all interactions with clients.
- General
Duties: These key responsibilities are not all inclusive, and are subject to change at the discretion of the CEO.
Other responsibilities may be assigned as necessary to meet the needs of the organization.
All WRM positions are subject to a background check to ensure compliance with insurance requirements.
QUALIFICATIONS
- Bachelor's degree in social work, nonprofit management, public administration, or a related field required; Advanced degree is highly desirable.
- Minimum of two years of experience in case management or related roles, preferably in a nonprofit organization.
- Understanding of case management best practices, social services, and proficiency in case management software.
- Strong alignment with Winchester Rescue Mission's faith-driven core values.
- Excellent leadership, communication, and interpersonal skills, with the ability to inspire and motivate team members to achieve organizational goals while upholding the organization’s mission and core values.
- Outstanding written and oral communication skills; strong problem-solving and decision-making skills; ability to prioritize tasks and manage time effectively; ability to work independently and as part of a team; flexible and able to handle a wide range of tasks; discreet, diplomatic, displays strong interpersonal skills.
- Excellent administrative and organizational skills with strong attention to detail.
- Good understanding of Winchester and the surrounding area.
- WORKING CONDITIONS
Part-time or Full-Time hours with some weekend hours per CEO discretion.
Work location is 435 N Cameron Street or 2655 Valley Avenue in the City of Winchester.
Regular attendance at Mission meetings and training sessions are required.
