Key Responsibilities
- Provide tailored support plans for patients based on their diagnosis.
- Arrange ongoing, long-term support for patients post-care.

Essential Qualifications
- We are looking for a Registered Mental Health Nurse (Band 5) with NMC Registration.
- At least 2 years post-graduate experience.
- 6 months recent NHS experience within the last 3 years.
- The ability to pass an enhanced background check.
