Actively support patients and families , communicating clinical information and treatment options to ensure practical understanding. Facilitate shared decision-making with patients and the multidisciplinary team. Lead or work within a multidisciplinary team to manage patient care. Prioritize patient/client requirements , delegating effectively to others. Recognize and report changes in a patient\’s condition to the team. Perform basic administrative tasks such as keeping records, writing prescriptions, and monitoring treatment plans. Teach and train junior doctors , medical students , and other team members .
