Together First CIC in London is recruiting a Neighbourhood Care Coordinator to support integrated, person-centred care across health, social care, and voluntary services. You will work with GP Neighbourhood Representatives, Primary Care Networks and MDTs to coordinate care pathways for patients with complex needs and ensure timely access to services.

Your role includes:

  • Managing MDT meetings
  • Coordinating referrals and caseloads
  • Enabling personalised care planning
  • Facilitating communication
