The role

Do you have what it takes to lead an award-winning integrated outreach team for people experiencing homelessness in Southwark, committed to developing new ways of supporting and engaging with people with multiple disadvantages? This post is responsible to both the SLAM Trust Homelessness Service Lead and Mental Health Service Managers from Adult Social Care. You will lead a small integrated multidisciplinary team. You will be based within a friendly, creative, collaborative, and supportive department at Trust offices in Kennington, with access to council offices in Camberwell.

The team offers wrap-around, multidisciplinary support to clients, including statutory social work, nursing, psychiatry, and peer navigation, working closely with third-sector partners in the borough, including day centres and St Mungo’s. You will act as a bridge between statutory services, including health and housing, and charitable organizations to ensure high-quality support for people with complex needs who often cannot access mainstream services. The team aims to deliver outreach-based, relational care to service users, working to improve their quality of life across all Care Act domains, supporting transitions into stable accommodation, and reducing the likelihood of returning to homelessness.

The role requires promoting safe, innovative, and creative approaches to meet the needs of both service users and carers, in line with best practices and quality assurance, alongside performance and budget management, and resource oversight.

About you:

You will be a registered Social Worker with experience managing teams within health, adult social care, or voluntary sectors. You will possess excellent communication, organizational, and partnership development skills, with proven leadership experience to develop high-quality services for Southwark residents experiencing homelessness, both on the streets and in hostels. You will have a genuine passion for supporting people experiencing homelessness and multiple disadvantages, demonstrating a commitment to their recovery through relational care and advocacy. You will be knowledgeable about current social care legislation, policies including safeguarding adults, and principles of equality and diversity, applying these in practice. You are a practical thinker capable of managing demand while keeping the person at the center of strategic planning.
