About the Role:

Cheshire East (Macclesfield) Council is seeking a dedicated Social Worker to join their Hospital Discharge Team. This role involves ensuring smooth transitions from hospital to home, making a meaningful impact on patients’ lives.

Responsibilities:

• Assess individuals\’ needs upon hospital discharge to develop tailored care plans promoting independence and well-being.
• Collaborate with hospital staff, families, and external providers for effective discharge planning and support.
• Conduct risk assessments and follow safeguarding protocols to protect vulnerable adults.
• Implement legislation such as the Care Act and Mental Capacity Act to ensure compliance.
• Facilitate access to community resources and support reintegration into home or care settings.
• Maintain accurate records, participate in supervision, and contribute to service improvements.
