An exciting opportunity has arisen for an experienced Support Worker (RSW) to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS) in the St Andrews Locality.
The Intermediate Care Service provides rehabilitation to the adult population within the locality. This will be delivered to service users as in-patients, out-patients, or within their own homes.
The primary function of this role will be to work with Occupational Therapy, Physiotherapy, and District Nursing Services within Intermediate Care to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital. These services will provide specialist assessments and allocate work accordingly. This post will primarily focus on daily care in the community, a service provided 365 days a year from 0800-2200.
You will have an SVQ Level III in Health and Social Care or equivalent. Experience working as a support worker is desirable. Experience working with older people is also desirable, as they are the main group of service users referred. Working as part of a supportive multidisciplinary team, you will carry out ongoing treatment programmes for a mixed caseload, mainly consisting of older adults.
You should have good organisational skills, excellent communication skills, and the ability to work independently as well as within a team. There may be occasions where you will be required to work at other community hospital sites. You will need to work 15 hours flexibly over 7 days.
Our service offers regular supervision, Personal Development Planning (PDP), and training opportunities. The ability to travel throughout the area is essential, as this is a community-based post.
