You will work within Shropshire’s People’s Directorate, with children, adults, and families. The role involves working with children on the edge of care, children in care, and supporting children and young people to remain in, or return to, the care of parents and family where it is safe and in their interests, through support to the adult. As an Outreach Worker, you will assist in delivering intensive family support to children, young people, and their families where there is a risk of family breakdown (Edge of Care). An Assessment of Needs will identify support needed to prevent unnecessary reception into Local Authority care. You will also support young people to make successful transitions into independent living from care, and help facilitate stepdowns from foster care or residential placements back to the family home. Additionally, you will support parents in making changes necessary for children or young people to return home.
