To assist registered nurses/practitioners in delivering care to patients/clients, working in partnership with other healthcare professionals, patients, their families, and carers. Deliver patient care as documented in the nursing care plan. Carry out a wide range of duties under the direction of a registered nurse during the shift.
An opportunity has arisen to join the highly motivated District Nursing Service in the Conwy Locality. The role involves supporting the district nursing team in caring for those living in the community who require nursing care. Applicants should be enthusiastic, aware of, and committed to the modernisation of the service. They should be able to work well within a team and independently.
You must be able to travel between patients' homes in both towns and rural areas in a timely manner. Welsh speaking skills are desirable.
