Job overview
The post holder will provide managerial leadership for Continuing Health Care (CHC) teams across Tower Hamlets and Newham. The post holder will be the catalyst bringing together two teams in one space, ensuring that each team meets their individual contractual obligations. They will be accountable for ensuring teams have appropriate staffing levels, including the right skill mix to meet the demands of Tower Hamlets and Newham.
They will work openly and transparently, embedding succession planning at every level.
The post holder will be an expert CHC clinician with extensive CHC knowledge demonstrated by extensive experience. They will demonstrate expert knowledge of CHC policy at local and national levels, providing assurance to the lead nurse that teams are competent and proficient in delivering this highly specialised care.
Main duties of the job
- Act as a subject matter expert ensuring that national & local KPIs are met
- Ensure the team meets Trust/regulatory body requirements
- Undertake regular training needs analysis reviews, providing or commissioning training to meet service demand
- Ensure the application of the CHC Framework to the highest standard for fully funded/joint funded patients
- Act as the Adult Safeguarding lead in the service, working collaboratively with lead nurses and local authorities
- Conduct regular service reviews, keeping the patient at the center of care
- Lead the introduction of evidence-based packages of care, developing interdisciplinary care pathways, protocols, and audits
- Provide expert clinical advice to service users, carers, and colleagues
- Undertake quality assurance activities including research and patient/family surveys

Detailed job description and main responsibilities
- Provide expert clinical guidance to the Adult Continuing Healthcare Panel
- Lead case management ensuring evidence-based care is embedded
- Lead KPI-compliant CHC assessments
- Ensure nursing and CHC assessments are completed to the highest standards, implementing action plans for improvement
- Represent CHC teams at local and national panels and meetings
- Collaborate with healthcare providers and care homes, reviewing individual cases for eligibility
- Lead the review of CHC cases for funding eligibility as required
Person specification
Education/Qualification/Training
- Registered nurse on Part 1 of the NMC register
- Relevant postgraduate qualifications at Masters level or working towards this
- Non-medical prescribing
- Expert knowledge in continuing health care, with evidence of ongoing professional development
Knowledge, Skills and Experience
- Significant post-registration experience managing large teams
- Current, active senior clinical practitioner within NHS Continuing Healthcare, with expert knowledge of policies and guidelines
- Experience in partnership working and cross-organisational staff management
- Responsible for safe use of specialist equipment related to continuing care
- Experience in change management and facilitating groups
- Skilled in planning and organising complex activities, including delivering training
- Experience working within diverse communities
