Wards Pharmacist

Overview

Mediplacements are looking for an experienced band 7/8A Wards Pharmacist to work at a hospital in Surrey. The pay for this position is GBP28:GBP36 per hour, and we are looking for someone who can start as soon as possible for an on-going duration.

As a ward pharmacist, you will be responsible for providing pharmaceutical care and medication management to patients admitted to hospital ED wards.

Responsibilities

• Provide high quality, patient-centred clinical pharmacy services across a range of specialties.
• Deliver expert pharmaceutical care, medicines optimisation, and evidence-based advice to patients, carers and multidisciplinary teams.
• Undertake medicines reconciliation and screen prescriptions.
• Support safe and cost-effective prescribing, facilitate timely discharge, and ensure seamless transfer of care across sectors.
• Contribute to clinical governance and support the development of guidelines.
• Supervise trainee pharmacists and Band 6 pharmacists.
• Participate in audits and quality improvement initiatives.
• Work with rotation leads to enhance services.

Qualifications

• MPharm degree.
• GPhC registered.
• Experience working in a hospital ward setting.
