Social Worker : Mental Health Discharge Team

Sirona Medical is recruiting for a Social Worker – Mental Health Social Work Discharge Team.

The role will involve delivering a statutory social work service to those people who access or need specialist mental health services and will support their discharge from hospital. You will work in partnership with the newly formed mental health hospital discharge hub, including the completion of all statutory work such as Care Act assessments, s117 aftercare planning meetings, safeguarding, Mental Capacity Assessments, and Best Interest Meetings, as well as any other social care related tasks and functions.

Pay: £38 per hour Ltd. Or £35 with £150 weekly expenses (for those living outside of the Authority).

Working arrangements: Mix of both – office based 3 days a week and the rest from home.

Successful applicants will have:

1. Fully qualified (DipSW, CQSW, CSS or equivalent).
2. Social Work England Registered.
3. At least 3 years post qualified experience.
4. Approved Mental Health Practitioner (it is a requirement to have AMHP qualification or to be working towards the qualification to be completed within 2 years of appointment and to maintain level of knowledge).
5. Full, valid driving licence.
